Capo Di Tutti Capi  = boss of all bosses  ;)

boss is really a recent thing in the mafia, since it only came into play when sicilians came to america, as before the position was refered to as father, and they had a paternal kinship orientated relationship with leader and his followers. boss denotes a more master and servant/worker relationship which is probably why the mafia isn't doing so well any more, as there is less loyalty in that kind of relationship.

As Ka said, Capo is a recent invention, but basically, modern mafia organization is this, within each family:

Don-same as always, Godfather is a common title

Capo-captain, lieutenant, etc...a middle ranker..Clemenza, Tessio, etc

Mademen-Luca Brassi, etc...junior members of the family, no title, but they're respected over the hired hands
