I gotta agree with ONE LIFE by Last Emperor ft. Poetic of Gravediggaz, it doesn't get anymore real than this.   Having a guy rap about dying when he knows he only has a couple months to live is incredibly powerful, and trhe beat fits perfectly.  Incredible song, if you haven't heard it you need to.
